PROCEDURE FOR APPLICATION FOR SPECIAL CLASS

Strictly for Graduating Students ONLY

  1. Have all credited subjects with their corresponding grades verified and evaluated at the Registrar’s Office.
  2. Secure endorsement form from the Registrar’s Office, duly notified by the authorized staff and attested by the School Registrar.
  3. Write a letter of request for special class/es with the ff. attachments noted by the Area Chairperson and addressed to the College Dean for approval:
    3.1 Duly signed endorsement form by the Registrar’s Office
    3.2 Registration Form
  4. Have the letter checked and noted by the Area Chairperson.
  5. Seek for the Dean’s approval of the request.
  6. Pay at the Finance Office the amount corresponding to the requested number of special classes/units approved by the Dean.
  7. Submit to the Registrar’s Office the approved letter of request together with the proof of payment.
  8. Follow-up from the Dean’s Office or from the Academic Coordinator as to who will be the faculty member/s to handle the special class/es requested.
  9. Attend/Report to the special class/es as scheduled.
