PROCEDURE FOR APPLICATION FOR SPECIAL CLASS
Strictly for Graduating Students ONLY
- Have all credited subjects with their corresponding grades verified and evaluated at the Registrar’s Office.
- Secure endorsement form from the Registrar’s Office, duly notified by the authorized staff and attested by the School Registrar.
- Write a letter of request for special class/es with the ff. attachments noted by the Area Chairperson and addressed to the College Dean for approval:
3.1 Duly signed endorsement form by the Registrar’s Office
3.2 Registration Form - Have the letter checked and noted by the Area Chairperson.
- Seek for the Dean’s approval of the request.
- Pay at the Finance Office the amount corresponding to the requested number of special classes/units approved by the Dean.
- Submit to the Registrar’s Office the approved letter of request together with the proof of payment.
- Follow-up from the Dean’s Office or from the Academic Coordinator as to who will be the faculty member/s to handle the special class/es requested.
- Attend/Report to the special class/es as scheduled.
