PROCEDURE IN REQUESTING FOR TRANSFER CREDENTIALS

(For those who have already enrolled officially for the current semester)

  1. Prepare a personal letter of request (either computerized or typewritten) for Honorable Dismissal, indicating your reason for such. Address the letter to: College Dean
  2. Have the letter:
    2.1 signed by the student applying for the honorable dismissal and by his/her parent or guardian
    2.2 noted by the Area Chairperson
    2.3 Approved by the Dean of College
  3. Forward the letter of request to the Guidance Center where the exit interview is duly scheduled and conducted. The College Guidance Counselor will take note of the student’s reason for request for honorable dismissal and sign letter once the exit interview is completed.
  4. Submit the said letter of request to the Registrar’s Office.
  5. Follow-up status of request within the week. It is only then when a blank clearance form will be issued for honorable dismissal.
  6. Accomplish the clearance form completely, making sure that it is signed by the actual persons in charge of the respective offices or by their duly appointed representatives. Forged signatures will lead to non-issuance of the academic credentials.
  7. Present the complete accomplished clearance form to the Registrar’s Office for processing. Follow up three weeks later.
  8. Pay corresponding fees and present the official receipt to the Registrar’s Office when claiming your records.
